Unless you have been notified that your child is on an MDSL roster for the spring season, please fill out the $40 option for payment. If you have been notified that your child is on an MDSL roster for the spring, please fill out the $60 option for payment. The only teams that will be in MDSL are the 2012, 2014, and 2015 Premier teams.
